The Grand Rehabilitation and Nursing at Barnwell, located in Valatie, NY, is currently seeking Evening LPNs to join our team!

Position Overview:

As an Evening LPN, you will be responsible for delivering exceptional patient care by following nursing processes, administering treatments, and addressing both physical and emotional needs of our patients. You will work closely with our CNA team to ensure that care is delivered efficiently and accurately, while also managing medication distribution and maintaining detailed patient documentation.

• This position can be full-time or part-time

  Hours: 3:00pm to 11:00pm


• 
  Base Pay Range: $27.00 to $34.00 per hour
  Shift Differential: Evening Shift: +$2/hr

LPN Responsibilities:

• 
  Patient Care & Service: Deliver exceptional care by completing the nursing process, performing treatment procedures, and offering physical, emotional, and educational support to patients, families, and visitors. Supervise and guide assigned team members.


• 
  Documentation: Accurately complete and maintain resident care documentation using a computerized medical record system.


• 
  CNA Support: Lead the CNA team in providing quality, individualized personal care and ensure that all CNA documentation is correct and up-to-date.


• 
  Medication Administration & Reporting: Administer prescribed medications and provide verbal and written reports to the appropriate team members following departmental protocols.

LPN Requirements:

• Current and valid NY LPN License


• Previous experience in a nursing home or hospital setting is a plus


• Valid CPR Certification
