Position Overview:
We are currently seeking compassionate, skilled LPNs to join our team. In this role, you will provide high-quality care, perform nursing treatments, and meet the physical and emotional needs of patients. You will also collaborate with our CNA team to ensure timely and accurate care, while managing medication administration and keeping thorough documentation.

LPN Responsibilities:

• 
  

  Patient Care: Promote and restore patients' health by performing nursing assessments and treatments. Provide physical, emotional, and educational support to patients and their families. Supervise assigned team members to ensure high-quality care.

  Documentation: Maintain accurate and complete patient care documentation using our computerized medical record system.

  
  


• 
  

  Team Support: Collaborate with CNA team members to ensure individualized personal care is delivered in a timely manner, with accurate documentation.

  
  


• 
  

  Medication Administration & Reporting: Administer medications and relay pertinent information to staff, following departmental protocols.

  
  

Requirements:

• 
  

  Current, valid NY LPN License

  
  


• 
  

  Experience in a Nursing Home or Hospital setting is a plus

  
  


• 
  

  Valid CPR Certification
