Evening Support Home Health Aide (HHA) – Full-Time
Department: Clinical Services
Schedule: Monday–Friday, 3:00 PM–10:00 PM
Service Areas: Skilled Nursing Facilities, Assisted Living, Memory Care Units, Independent Living, Private Residences
Compensation: $20–$21 per hour

About the Role
As an Evening Support Home Health Aide (HHA), you’ll be a vital part of our Moments That Matter Program—an initiative that nurtures emotional wellness and personal comfort. This role integrates group engagement in care facilities with individualized patient visits, ensuring an atmosphere of warmth, relaxation, and meaningful interaction.
Key Responsibilities:
Facility Program
Lead group evening activities at assigned facilities- tailored to all residents (hospice and non-hospice), fostering connection and emotional well-being.
Create a soothing environment using music, aromatherapy, and comfort-based approaches.
Provide gentle wellness checks, hydration support, and light physical assistance.
Facilitate reflective and memory-enhancing activities that support emotional health.
Collaborate with facility staff and caregivers to ensure a meaningful experience.
Patient Program
Deliver personal care visits in homes and facilities, focusing on comfort and companionship.
Support new hospice patients through their initial 30-day transition with scheduled visits.
Offer presence and emotional support during end-of-life phases, increasing visit frequency as needed.
Monitor and report patient conditions promptly to clinical teams.
Assist with personal care tasks aligned with individual needs and care plans.
Requirements
Qualifications & Skills
Certification: Current Home Health Aide (HHA) certification in good standing (state-specific).
Experience: Previous hospice, palliative, or elder care experience strongly preferred.
Engagement Skills: Ability to lead group activities and foster connection.
Professionalism: Demonstrated emotional maturity, patience, and sensitivity in care.
Dependability: Reliable transportation and flexibility to travel between locations.
Availability: Evening hours required.
