Job Description - Facilitator/Case Manager

Description

POSITION OVERVIEW:

The Facilitator/Case Manager will be responsible for facilitating clients in the Strengthening Families and Youth and Family Services curriculums.

The Case Managers and families will collaborate in developing the case plan by identifying specific services/resources needed. The Case Manager will ensure referral linkage to community resources appropriate to meet the client’s needs, engagement and then follow-up to ensure they are able to access the service and that the service was helpful. Case Managers will assist in determining eligibility for health insurance and other benefits specific to their situation and facilitate the enrollment process. The frequency of case management will be customized depending on the needs of the client but case management services are expected to last an average of 10 months. Case Managers will carry a client to Case Manager ratio of approximately 35-45 active participants and have access to the vast network of current MHA partners as well as have access to the 30+ programs available through MHA such as housing, transportation and outpatient services to ensure a comprehensive approach. 

ESSENTIAL POSITION RESPONSIBILITIES:

  1. Participates in planning sessions with clients to develop a plan for project participation and to refer, link, and engage fathers for needed community services.
  2. Provide monthly review of case to monitor progress towards goals, and makes revisions as deemed appropriate.
  3. Provides supports as identified by the case plan.
  4. Provides information, trains, and offers general assistance in filling applications for all community-based resources.
  5. Provides assistance with the following: family dynamics, education, employment activities, benefits, acquisition, housing, problem solving/crisis management, and economic stabilization.
  6. Maintains accurate, legible documentation of contact with clients and completes activity logs within 24 hours of the delivery of service.
  7. Provides ongoing consultation and support to clients during the duration of their open case to meet the needs of the family and the program.
  8. Assist in recruitment of clients.
  9. Facilitate Strengthening Families curriculum and Parenting for Generations curriculum.

OTHER POSITION REQUIREMENTS:

  1. Maintains acceptable overall attendance record, to include department staff/supervision meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences, and ensures that work is covered. Flexible in work schedule when needed.
  2. Exhibits appropriate level of technical knowledge for the position.
  3. Works well with a team, keeps others informed of information needed. Treats others with respect, maintaining a spirit of cooperation.
  4. Demonstrates the ability to access family needs.
  5. Maintains effective and professional verbal and written interactions with peers, fathers/families, supervisors, and other staff. Uses diplomacy and tact in dealing with difficult situations or people. Demonstrates effective listening skills. Is receptive to constructive feedback.
  6. Demonstrates the ability and willingness to handle new assignments, changes in procedures, and business requirements. Identifies what needs to be done and takes appropriate action.
  7. Performs work conscientiously with a high degree of accuracy.
  8. Meets deadlines and ensures accuracy of various reports/paperwork, mileage records, and electronic timesheets.
  9. Completes assigned work, meets deadlines without reminders/follow-up from supervisors/others.

POSITION REQUIREMENTS: The Facilitator/Case Manager position will require an ability to connect fathers to community resources, housing alternatives and vocational services, and mental health and substance abuse resources; skilled at problem solving and helping fathers through crisis; group facilitation skills and the ability to develop and maintain rapport with constituents and staff. A valid driver’s license and access to personal vehicle required and be able to travel the 10 county Wichita Region as needed. 

PHYSICAL REQUIREMENTS:

* Driving (for purposes of community mobility)

* Typing/data entry, writing/note taking

* Lifting/carrying up to thirty (30) pounds

* Bending/Stooping/Climbing stairs
