Facility Transition Planner

Position Summary: 
North South Consulting Group is actively recruiting a detail-oriented Senior Transition Planner to manage the seamless transition and relocation of staff, equipment, and operations into a newly constructed VA facility in Elizabethtown, KY. This role is essential to ensure that clinical operations begin without disruption, while maintaining alignment with VA transition protocols, timelines, and stakeholder expectations. 

Key Responsibilities: 

• Lead transition planning for all impacted departments, including logistics, staffing, relocation, and operational readiness 

• Develop detailed “Day in the Life” plans and department-level move sequencing documentation 

• Coordinate relocation of FF&E, personnel files, and supplies in accordance with the Move Plan 

• Collaborate with VA stakeholders to assess existing operations and define transition requirements 

• Prepare integrated move plans and ensure compliance with security, safety, and continuity protocols 

• Facilitate the development of employee orientation and welcome materials 

Required Qualifications: 

• U.S. Citizenship (Required) 

• A minimum of three (3) years of experience in transition planning and relocation services on multi-phased healthcare projects  

• Experience in healthcare setting  

• Demonstrated experience in conceptual advance planning, identifying occupancy milestones, detailed implementation planning and move sequencing, preparation and setup, compiling documents of detailed move plans for each department impacted, and relocation execution  

• Demonstrated experience in multi-phased healthcare projects involving space activation or organizational change 

• Proven ability to develop and execute transition and move plans for large facilities 

• Must be able to obtain a favorable background investigation suitable for VA facility access 

Desired Qualifications: 

• Prior experience supporting VA facility activations or similar federal transitions 

• Familiarity with VA's OCAMS SharePoint tools 

• Experience managing CONOPS planning, transition playbooks, and department-level workflows 

• Strong stakeholder communication and coordination skills
