JOB SUMMARY

The Family Case Manager will provide trauma-informed, guest-driven case management services to individuals who are experiencing homelessness with the goal of helping them achieve stable housing solutions quickly and efficiently. The services provided include identifying and engaging with individuals in our Resource Centers and connecting them with resources in the community that will aid in their transition into housing. The Housing Focused Case Manager will work within the shelter team to identify common barriers faced by individuals in shelter and help with establishing strategies and procedures for addressing needs on a shelter-wide scale.

Grade 6/$18.90/hour

Put ‘Housing First'

You will:

  • Provide services to families experiencing homelessness in an emergency shelter.
  • Assist guests in identifying barriers and connect them with appropriate resources in the community.
  • Strong belief in the Housing First model.
  • The ability to engage and encourage housing conversations with guests.

Work with a Case Load

You will:

  • Assisting our diverse population of guests on your set caseload to establish goals.
  • Assist families in obtaining housing and or working on barriers into housing.
  • Focus on housing and strengths-based practices to meet the unique needs of each client.
  • Assess family's needs to make appropriate community referrals.
  • Daily collaboration with all departments to promote a healthy team environment.
  • Seeking out, engaging, and networking with community partners to find housing options and additional resources.
  • Attended weekly team meetings and contribute to case conferencing conversations.
  • Keep accurate records, case management notes, and statistical data as required and participate in special projects as needed.

Resource Collaboration

You will:

  • Provide direct, trauma-informed case management services to clients, including ongoing assessments, goal setting, and creative problem solving, using a network of community resources.
  • Ability to accept direction and constructive feedback from Supervisor.

Promoting Best Practice

You must:

  • Demonstrates awareness and desire to understand diverse populations
  • Ability to contribute to the Agency’s commitment to enhancing awareness and appreciation
  • Willingness to understand that housing first, harm reduction, low barrier access to resources is best practice and essential to our mission at The Road Home.
  • Ability to set and maintain good boundaries, using Trauma-Informed Care as a guideline
  • Other duties as assigned
  • Ability to pass a pre-employment background check and drug screen.

Required Skills and Abilities

  • Strong interpersonal skills and ability to work with diverse populations.
  • Demonstrated awareness of and sensitivity to diverse populations. Ability to contribute to the agency’s commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.
  • Excellent writing and documentation skills.
  • Willing to be a proactive member of a team.
  • Ability to work from a Trauma-Informed Care framework.
  • Familiarity with the Housing First philosophy.
  • Ability to work with community partners.
  • Self-starter and self-directed.
  • Ability to accept direction from Supervisor.
  • Excellent organizational skills and strong computer skills.
  • Must pass a pre-employment background check and drug screen.

Education and Experience

  • Lived, work or volunteer experience preferred or a degree in a related field
  • Experience working or volunteering with diverse populations

Physical and Equipment Requirements

  • Ability to move 25 pounds.*
  • Able to walk and stand for at least an hour at a time.*
  • Licensed and able to drive a vehicle.*
