Family Centered Treatment Coach/Transitional Housing Case Manager

Description

The purpose of this position is to provide Family Cenetred Treatment (FCT) in the families home and community, to both foster families and parenting youth in transitional housing. FCT is evidence based and committed to family preservation, empowering families to set and achieve their own goals utilitzing a wide range of researched based interventions.The services of the coach will be based on an individualized case plan developed for each family. Specific responsibilities and duties will vary depending on the case/clinical plan, needs of child, and needs of foster parents.

The case manager will be responsible for working with up to 6 non minor (NMD) dependents and/or a ratio of up to 5 pregnant or parenting NMD. Responsible for coordinating with significant others, placing agency, and adjunct treatment members to enable the NMD to attain his or her goals. The case manager will not be responsible for providing after hour assistance to clients or on-call services. 

Requirements

•  Candidate must have a minimum of a bachelor’s degree in social work or a closely related field, as defined in the State Department of Social Services, Community Care Licensing regulations with at least 1-year prior experience working with the population 
•  Must possess valid California Driver’s License and have a good driving record.
• Fingerprint and Child Abuse Index clearances from the Department of Justice.
•  Must meet all requirements set by Community Care Licensing pertaining to the hiring of employees by a licensed community care facility.
