Full Time Position - 40 hours per week

Salary Range - $53,000 - $56,000

General Summary:

The Program Manager, Family Connects NJ (FCNJ) supervises program support specialists and coordinates the enrollment of eligible families in the FCNJ program. Assist with hospital partnerships through bedside recruitment and multidisciplinary team referrals. Works in collaboration with FCNJ team leadership.

Essential Functions:

Contribute to The Cooperative’s commitment to equity and continuous quality improvement that align with the agency’s mission, goals, and advancement * Effectively cross-collaborate with agency staff, leaders, and board members to foster a comprehensive approach of information sharing, working committees, and collective representation  * Routinely disseminate relevant information, presentations, trainings, and learning opportunities for internal program and agency staff * When applicable, will participate in relevant staff development opportunities (trainings, summits, conferences, etc.) to increase skill development, elevate program goals, and engage staff members with timely key learnings * Supervises FCNJ Program Support Specialist team, with input from the Director of Nurse Home Visitatio * Approves and signs off on monthly expense reports, bi-weekly time sheets, and PTO requests. * Completes staff performance reviews for the evaluation process with input from Director of Nurse Home Visitation. * Coordinates orientation for new FCNJ staff * Coordinates staff education, training and the implementation of new program directives, with the Director of Nurse Home Visitation * Conducts quality reviews including review of documentation  * Compiles data for reports, completes reports and submits to the Director of Nurse Home Visitation * Represents FCNJ on The Cooperative and DCF NJ committees/meetings, as directed by Director of Nurse Home Visitation * Performs other duties as assigned


Requirements

  • Bachelor’s degree in social work, counseling, nursing or other health-related field required
  • Demonstrated leadership skills
  • Organizational skills, problem solving skills with ability to prioritize work requirements
  • Must present in a professional manner and be able to engage with a wide range of clients and professionals
  • Ability to work independently and as a team member with good leadership, oral and written communication skills
  • Valid driver’s license, current insurance policy, and car

Physical Requirements:

  • Must be able to see to read documents and hear well enough to communicate with others
  • Must be able to write, type on a computer, and use a telephone
  • Must be able to operate a motor vehicle
  • Must be able to lift up to 10 pounds
  • Additional requirements to be determined with input from Director and HR

Work Environment:

Travels regularly to sites where traffic, weather, and other environmental conditions may be encountered; otherwise works in a climate-controlled professional office environment generally free from hazards and other adverse working conditions or remote home location.

Health and Wellness:

Our hospital partners require up-to-date vaccinations which include MMR immunity, Varicella immunity, and PPD testing. You may be required to provide vaccination record, obtain bloodwork to verify titers, and other testing to comply with hospital requirements.
