Description

The Iowa Primary Care Association is actively recruiting a Family Med Nurse Practitioner or Physician Assistant to provide care at a vibrant community health center (FQHC) in West Burlington, Iowa!

What is a community health center (FQHC)?

The largest source of comprehensive primary care for underserved communities and populations.  

With an integrated care model that includes medical, dental, behavioral health, and pharmacy services, patients can access comprehensive, coordinated services often under one roof. 

Role Details:

  • Clinic Hours: M/T/W/Th: 7AM - 6 PM; Fri: 7am - 5 PM; no weekends (schedule within these hours will be discussed) 
  • Full time position: 36 patient contact hours, 4 admin hours
  • Sees on average 18-20 patients per day
  • Provider team includes: 2 physicians, 3 NPs
  • EHR: Ochin Epic

Duties:

  • Provides client education to promote wellness and support disease management process.
  • Prescribes and dispenses medications.
  • Document accurately all assessments, interventions, client/family responses, medication dispensed or prescribed, procedures, treatments and other data in medical record.
  • Communicates test results and follow-up to patient.
  • Provides or participates in education for the client and family by assessing learning needs, formulating a teaching plan and initiating a teaching plan to meet the needs of the client.
  • Coordinates client care by triaging phone calls regarding urgency of appointments, and schedules hospital admissions and testing.
  • Maintains knowledge of available community resources.
  • Uses all available resources for diagnosis and treatment in an appropriate and cost efficient manner.

  • Competitive Salary: $110,000 - $151,000 based on experience, plus quality and productivity incentives
  • Additional Compensation Opportunity: the health center is planning to open a walk-in clinic in September that will provide the opportunity to earn additional compensation for picking up days beyond the normal schedule (i.e. standard schedule of four 10-hour days and could pick up day 5 in the walk-in clinic for additional dollars + those patient visits counting toward productivity bonus).
  • Contract Bonus: Sign a 3-year contract and receive a $5,000 bonus each year
  • Licensing and Professional Subscriptions: Reimbursement provided.
  • Comprehensive Insurance: Medical, dental, vision, life, accidental death and dismemberment, short-term disability, long-term disability, and flexible spending available.
  • Wellness Reimbursement Program: Reimburse your monthly gym/fitness/wellness fee up to $25/month
  • Continuing Education: 5-10 paid days and $1,500-$2,700/year based on experience
  • Paid Time Off: 20-28 PTO days per year, plus 7 paid holidays.
  • Retirement Plans: 403b retirement plan – 4% of salary plus an additional 1% if the employee contributes 2%; 457 plan eligible
  • Malpractice Insurance: FTCA Malpractice Insurance coverage

Requirements

  • BLS certification
  • Mandatory Reporter for Child and Dependent Adult Abuse
  • Clinical experience preferred

Nurse Practitioner:

  • Graduation from an accredited/approved NP program
  • Licensed by the Iowa Board of Nursing as a NP

Physician Assistant:

  • Graduation from an accredited AMA-approved PA program and/or certified by the National Commission on the Certification of Physician Assistants.
  • Licensed by the Iowa Board of Professional Licensure; licensed to practice in Iowa.
