JOB OVERVIEW

• Job Title: Physician – Family Medicine

• Job Type: Full-Time, Permanent (Employed)

• Location: Hobart, IN

• Service Setting: Outpatient Clinic

• Coverage Type: Clinical + Call

• Coverage Period: Ongoing

• Clinical Shift Schedule: Monday–Friday

• On-Call Shift Schedule: 1:5 call rotation

• Call Type: Rotational (newborn rounding only)

• Call Ratio: 1:5

PATIENT INFORMATION

• Patient Demographics: All ages; newborn rounding only

• Patient Volume/Census: 20–26 patients/day

• Admissions: No inpatient admissions

• Rounding: Newborn patients only

• Phone Consults: Yes

• Case Mix: General family medicine

FACILITY INFORMATION

• EMR System: EPIC – HIMSS Stage VII certified

• Office Equipment Available: Standard outpatient clinic tools

• Specialty Backup Available: Collegial group of system physicians

• Support Staff Available: Integrated support team

• Reason for Coverage: System expansion

PRIVILEGES & COMPLIANCE

• Credentialing Timeline: 90–120 days post-offer

REQUIRED PROCEDURES

• General family medicine care

• Newborn rounding

JOB REQUIREMENTS

• Licenses: Active IN license (or ability to obtain)

• Certifications: Board Certified or Board Eligible in Family Medicine, BLS preferred

• Experience: Open to new grads; support for practice development

• Other Qualifications: Comfortable in outpatient setting with EMR use (EPIC)

DUTIES & RESPONSIBILITIES

• Provide comprehensive outpatient family medicine care

• Manage 20–26 patient visits per day

• Participate in newborn rounding and call rotation (1:5)

• Maintain accurate documentation in EPIC EMR

• Collaborate with colleagues in a supportive, collegial environment

• Contribute to an expanding healthcare system with strong community ties
