Responsibilities:
- Provide primary care services to patients of all ages, including infants, children, adolescents, adults, and seniors
- Conduct comprehensive physical examinations and medical assessments
- Diagnose and treat acute and chronic illnesses
- Order and interpret diagnostic tests, such as laboratory work and imaging studies
- Prescribe medications and develop treatment plans
- Provide patient education on disease prevention, health promotion, and management of chronic conditions
- Collaborate with other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date medical records
Qualifications:
- Valid and unrestricted license as a Family Nurse Practitioner (FNP)
- Master's degree in Nursing with a specialization in Family Practice
- Minimum of 1 year of experience as an FNP preferred
- Familiarity with electronic health record systems (e.g., eClinicalWorks)
- Experience in utilization review and case management
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
- Ability to work independently and make sound clinical judgments

Our multidisciplinary outpatient facilities currently offer internal medicine, pediatrics and family practice. Our focus is on health care delivery with excellence, compassion and continuity. Our goal is to care for every family as we would care for ours. Routine vaccines are offered and school physicals.
We also offer nutritionist, health care counseling and weight reduction/management program. Among the routine tests we offer, are Diabetes Comprehensive evaluations, asthma and COPD action plan, influenza, mononucleosis, and Streptococcus screening. Routine vaccinations and antibiotic treatments are also given. Our staff members speak Spanish, and Portuguese.
We offer Ultrasound and Echocardiogram which are reviewed by board certified radiologists and cardiologist when findings are abnormal, Pulmonary Function Testing, Sleep Studies, DEXA Scan. BOTOX (botulinum toxin a) applications for Cosmetic and Therapeutic needs.
