Position Overview:
Greenlife Healthcare Staffing is currently seeking a Family Nurse Practitioner to fill an opening with a Community Health and Wellness Clinic located in Brooklyn, New York. This role focuses on providing comprehensive primary care to patients of all ages in a collaborative, patient-centered outpatient setting.

10:00 AM – 4:00 PM

Qualifications:
Education: Master's degree in Nursing from an accredited program.
Licensure: Active New York State Nurse Practitioner license, CME and DEA license.
Certification: Must be Board Certified as a Family Nurse Practitioner (FNP).
Experience: Strongly preferred 4 to 5 years of experience as a Nurse Practitioner.
Language: Bilingual Spanish is a plus.
Technical Skills: Proficient in performing physical examinations, ordering and interpreting diagnostic tests, prescribing medications, and utilizing electronic health records.
Soft Skills: Excellent communication and interpersonal skills, strong clinical judgment, empathy, patient-centered focus, and the ability to work collaboratively within a multidisciplinary team.
Key Responsibilities:
Perform comprehensive physical examinations and patient assessments for individuals and families across the lifespan.
Record and analyze patients' medical histories and symptoms.
Develop and implement individualized patient care plans, and contribute to existing ones.
Order, administer, and analyze diagnostic tests and laboratory results.
Diagnose and manage acute and chronic health conditions such as hypertension, diabetes, respiratory infections, and other common illnesses.
Prescribe medications and treatments as appropriate, including controlled substances.
Educate patients and families on disease prevention, lifestyle modifications, medication adherence, and treatment options.
Provide preventive care, including immunizations, health screenings, and routine check-ups.
Collaborate with physicians and other healthcare professionals within the family medicine practice to ensure coordinated, whole-person care.
Maintain accurate, thorough, and timely documentation in electronic medical records.
Provide compassionate, patient-centered care to a diverse patient population.
