Family Nurse Practitioner (FNP) is responsible for providing comprehensive medical care to patients, including diagnosing, treating, and managing medical conditions, prescribing medications, and providing health education and counseling. They would also work collaboratively with other members of the treatment team to ensure holistic patient care. The family nurse practitioner will be proficient in women’s wellness.
The FNP will be performing medical services consistent with SAMHSA and the American Society of Addiction Medicine (ASAM) Standards of Care. The FNP will report to the Medical Director in all matters of medical discretion, and for administrative purposes, to the Executive Director. The Medical Provider will ensure medical services are provided in compliance with federal and state regulations and are consistent with Company policies and procedures.
Job Duties
The FNP is ultimately responsible for overseeing, and directly or indirectly performing all medical and/or psychiatric management of company patients across multiple intensity levels consistent with the ASAM criteria (1-3) including but not limited to:
Provide primary care to members, including histories, exams, lab tests, prescriptions and referrals within prescribed up to date guidelines and protocols; including women’s health.
Assure that health education is provided with each patient encounter when applicable.
Timely coordination of care among internal and external ancillary providers and hospitals to provide high quality care between departments and facilities.
Assuming all patient care management duties for the Company, assuring each patient receives appropriate evaluation, treatment and management consistent with ASAM standards of care including but not limited to:
Completion of history and physical examinations of all new patients at detox and residential locations.
Completion of medical follow-up examinations for all detox patients daily, and for all residential patients weekly.
Completion of discharge management for all patients discharging from detox and residential locations.
Ordering and reviewing laboratory analysis (e.g. blood or urine drug testing) as indicated
Initiating and managing pharmacological treatment for patients with substance use and/or medical disorders.
Adhering to appropriate ASAM criteria (levels 1-3) for patient care with the goal of providing quality care at each intensity of service and ensuring safe and effective transition along the continuum
Collaboration with clinical staff to ensure determination and placement of patients at appropriate ASAM criteria (levels 1-3)
Participation in scheduled after hours “on-call” services to ensure 24/7 coverage for medical and/or psychiatric patient management
Participation in clinical supervision and/or training of medical staff and or development of treatment as needed or requested by Medical Director
Effective use electronic medical record system for patient management and documentation
Adhering to established Axiom formulary and treatment protocols as appropriate
Participation in continued medical education and/or in-service training as needed or requested
Participating in the development of a clinically relevant, outcome evaluation process
Collaboration with utilization review staff for purpose of justification for or appeal of determination and placement of patients at appropriate ASAM criteria (levels 1-3)
May require other duties as assigned.

Requirements
Possesses a current and unrestricted RN and Advanced Practitioner (FNP) license in the State of Arizona.
Possesses a current DEA license.
Credentialed with all AHCCCS (Medicaid) payors in Arizona.
Completion of an approved graduate medical education training program or demonstrated equivalent experience deemed supportive to the practice of addiction medicine.
Successfully satisfies criteria for credentialing.
Demonstrates leadership and a team focused practice approach. Possesses cognitive, organizational, emotional ability and maturity to effectively manage multiple stressors, deadlines and demanding situations.
Possesses outstanding written and verbal communication skills Requires willingness to accept additional responsibilities as needed.
There must be no history of alcohol or other drug misuse for a period of three years before employment.
CPR (BLS for the Healthcare Provider/Professional Rescuer or ACLS-the copy must include the front and back of card as applicable). Course must also meet the American Heart Association (AHA) standards.
At least three years of clinical experience as a Nurse Practitioner in ambulatory care, occupational health, family practice or emergency department setting preferred.
Knowledge of workplace health and safety concepts and OSHA regulations preferred.
Strong computer skills with knowledge of Internet software, Spreadsheet software and Word Processing software.
Knowledge and experience with Electronic Medical Records preferred.
