Job Description - Family Nurse Practitioner - FNP

We are seeking a dedicated and compassionate Family Nurse Practitioner to join our healthcare team. The ideal candidate will provide patient assessments and diagnoses. This role requires a strong commitment to patient well -being. The Nurse Practitioner will work collaboratively within a team and collaborate with physicians, nurses, therapists, and families.

Duties

Conduct comprehensive assessments of patients to determine their healthcare needs.
Offer ongoing support and education to patients and their families.
Collaborate with interdisciplinary teams to ensure excellent patient care.
Document patient interactions accurately in the electronic health record system.
Engage in continuous learning and professional development to stay current with best practices in medicine.
Proficiency in EMR/EHR systems such as Epic or Athena health; telehealth experience is highly desirable.
Excellent communication skills for effective patient assessment, family education, and interdisciplinary collaboration.

Requirements

Current Nurse Practitioner license - NM

3 Plus years of experience as a FNP
