Job Description - Family Partner

Must have lived experience as a caregiver of a child as a parent/guardian and/or foster parent.  

About Eliada Homes: Eliada Homes is a large non-profit child service agency dedicated to supporting children and families through comprehensive programs and services. Our mission is to help children succeed by providing a nurturing environment and addressing their behavioral, emotional, and educational needs.

Job Summary: The Family Partner will work closely with children and families experiencing behavioral issues, providing support, resources, and advocacy. This role is crucial in fostering a collaborative relationship between the child development centers and families to ensure the well-being and development of the children. The children in these programs are 6 weeks old up to school age.

Key Responsibilities:

· Support and Advocacy: Provide direct support to families, helping them navigate behavioral challenges and access necessary resources.

· Collaboration: Work collaboratively with childcare staff, therapists, and other professionals to develop and implement individualized care plans.

· Resource Coordination: Connect families with community resources, including mental health services, educational programs, and social services.

Documentation: Maintain accurate records of interactions, care plans, and progress reports.

Outreach: Engage in community outreach to raise awareness about Eliada Homes’ services and support available to families.

Requirements

  • Minimum high school diploma/GED, Preference will be given to individuals with Case Management experience, with Bachelor's degree preferred.
