Position Overview:
ONE Health Ohio is seeking a Physician to provides direct high-quality care in a primary care within a Federally Qualigied Health Center (FQHC). Physicians deliver comprehensive evaluations, diagnosis, and treatment to a diverse population of patients.
Key Responsibilities:
Provide comprehensive medical care, diagnosis, and treatment of acute and chronic illnesses and conditions.
Educate patients and families regarding diagnoses, treatment plans, preventive care, and safety, ensuring adherence to HIPAA and other regulatory guidelines.
Collaborate with a team including nurse practitioners, physician assistants and other healthcare providers.
Collaborate/supervise and mentor mid-level providers and students as needed.
Support collaborative, multidisciplinary care with clinicians, behavior health providers and other team members.
Provide patient education on health promotion, disease prevention, and management of conditions.
Provide ongoing follow-up, continuity care and chronic disease management.
Maintain accurate and timely documentation in the EHR record system.
Ideal Candidates Will Have:
Board Certified of Board Eligible
Graduated from an accredited school of medicine.
Licensed to practice medicine in Ohio with good standing
Current DEA certificate
Completion of accredited residency program.
Valid Ohio driver's license
Current BLS Certification
