Job Description - Family Physician (Full Time)


Reporting/Department Head: Chief Medical Officer

MAJOR FUNCTION: 

Provides medical care within the scope of NJ licensed Physician 

  • Assess the bio/psycho/social/spiritual health status of individual clients and families through history taking, data collection, and physical exam
  • Assists staff nurses to facilitate care for walk-in patients
  • Completes accurate documentation according to site specific standards
  • Works in collaboration with staff, physicians, allied health professionals, and others to implement the patients plan of care

Provides pharmacological care within the scope of a NJ licensed Physician

  • Participates in annual update of joint practice protocols as per NJ Board of Medical Examiners guidelines
  • Prescribes or make judgments regarding pharmacologic agents and therapeutic devices
  • Dispenses/Administers/Prescribes medications according to policy & protocol

Provides medical care within the scope of a NJ licensed Physician

  • Recognizes and manages deviations from health
  • Recognizes patients whose problem is out of scope of practice and obtain appropriate care
  • Orders appropriate laboratory and diagnostic tests ordered
  • Follows up on laboratory and diagnostic tests ordered
  • Communicates with patient to arrive at mutually agreed upon plan of care to include actual and potential health problems
  • Evaluation and revision of patient plan of care is ongoing

Provides health education

  • Informs, teaches and guides clients/families to assume responsibility for management of their own health
  • Individualizes education to facilitate patient independence and optimize patient self care
  • Educates patients on pharmacological agents, therapeutic devices, laboratory or diagnostic testing
  • Promotes health education for groups of patients, families, staff, agencies, communities, and organizations pertinent to area of specialty as appropriate to program

Provides referral and case management

  • Refers patients to internal resources according to program standards, protocol, or physician consultation
  • Refers patients for specialty care as per program standards, protocol, or physician consultation
  • Refers patients to primary care as appropriate
  • Follows up on referrals according to site specific standards
  • Case manages client care according to program standards and patients plan of care

OTHER JOB DUTIES: 

  1. Attends applicable leadership meetings and may help lead particular items in the clinical providers group which establishes and reviews clinical policies and practice standards.
  2. Supports CompleteCare in compliance with these policies and standards, helping to ensure that services are consistent with the standards set by various accrediting and governmental review organizations.
  3. As a representative of the providers at specific sites, some may be required to attend the Quality Performance Improvement (PI) Committee meetings to ensure compliance with the PI plan.
  4. Participates in meetings with the staff and providers at specific sites and contributes to the planning and execution of quarterly and annual all-staff meetings if requested.
  5. Helps in coordinating the specific site provider scheduling and coverage issues.
  6. Help conduct initial and annual performance evaluations, peer review and reviews needed to meet all requirements (PCMH, HRSA & NJ DOH).
  7. Provides clinical service consistent with job description for ObGyn
  8. Works with Health Center Manager to address operational issues, incident reports, or patient or provider satisfaction concerns
  9. Responsible for promoting and fostering an atmosphere of teamwork and excellence among staff.
  10. Recognizes the importance of financial standing and supports Senior Management by:
  11. ensuring that providers and sites achieve required productivity levels of 3.0 VPH by communicating expectations and actively supporting institutional goals of accessibility
  12. to support expected productivity through development and sharing of improved systems and operational efficiencies
  13. being a solutions-based and active leader
  14. Provider will receive four (4) hours of administrative time weekly, on average.

Requirements

QUALIFICATIONS:

  • Unrestricted license to practice as a Physician from the NJ Division of Consumer Affairs 
  • Employee shall maintain registration including prescriptive authority
  • Maintain Board Certification 
  • Maintain Active CPR
  • Ability to stand, swift waling, lifting, moving patients as needed
