Job Description

Position: Family Re-Entry Program Coordinator
Employment Type: Full-Time (pending position subject to contract award)
Contract Period: July 24, 2025 - June 30, 2026
Location: Salisbury, MD
Salary Range: Competitive, commensurate with experience

Position Overview

The Family Re-Entry Program Coordinator will lead the implementation and delivery of comprehensive re-entry services for parents and guardians being released from incarceration who have mental health and substance use disorders. This position requires an experienced professional who can provide direct services, coordinate care, and connect clients with essential community resources to support successful reintegration.

Essential Duties and Responsibilities

Direct Service Delivery

  • Provide peer support services to at least 75 program participants annually
  • Deliver care coordination services including assessment, planning, and follow-up
  • Provide comprehensive transportation services as detailed below
  • Conduct intake assessments and develop individualized service plans
  • Maintain regular contact with participants to monitor progress and address barriers

Transportation Services (Required Core Component)

The Program Coordinator must provide or arrange reliable transportation to ensure participants can access essential services and appointments.

Direct Transportation Provision:

  • Drive participants to medical and mental health appointments
  • Transport participants to substance use treatment facilities and meetings
  • Provide transportation to social service offices for benefit applications
  • Drive participants to employment interviews and job training programs
  • Transport to housing appointments and property viewings
  • Accompany participants to court hearings and probation meetings
  • Provide transportation to obtain identification documents and birth certificates

Transportation Coordination and Planning:

  • Assess individual transportation needs during intake process
  • Develop transportation plans that accommodate participant schedules
  • Coordinate with public transportation systems and community resources
  • Arrange shared transportation when multiple participants have similar destinations
  • Schedule transportation efficiently to maximize service delivery
  • Maintain transportation logs and mileage records for reporting purposes

Emergency and Crisis Transportation:

  • Provide immediate transportation during crisis situations
  • Transport participants to emergency medical or mental health services
  • Respond to urgent transportation requests within reasonable timeframes
  • Coordinate with emergency services when appropriate

Transportation Barriers Assessment:

  • Identify transportation challenges that may prevent service engagement
  • Address geographic barriers in rural or underserved areas of Wicomico County
  • Work with participants who lack personal transportation or driver's licenses
  • Accommodate participants with physical disabilities or mobility limitations
  • Address financial barriers to transportation costs

Vehicle and Safety Requirements:

  • Maintain a safe, reliable vehicle for transportation services
  • Ensure current vehicle registration, inspection, and insurance coverage
  • Follow all traffic laws and safe driving practices
  • Maintain vehicle cleanliness and professional appearance
  • Document vehicle maintenance and safety checks
  • Carry emergency supplies and first aid kit

Documentation and Compliance:

  • Record all transportation activities in participant files
  • Track mileage and transportation costs for budget reporting
  • Maintain confidentiality during transportation
  • Follow organizational policies for transporting participants
  • Document any incidents or concerns during transportation

Resource Connection and Advocacy

  • Assist participants with applications for social services including SNAP, Medicaid, MDRN assistance, disability benefits
  • Help participants obtain identification documents and birth certificates
  • Connect participants with housing assistance and employment opportunities
  • Link participants to substance use and mental health treatment services
  • Facilitate access to healthcare providers and coordinate medical appointments

Community Partnership Development

  • Establish and maintain relationships with community-based organizations
  • Create linkages to programs providing clothing, food, shelter, and emergency assistance
  • Collaborate with law enforcement, courts, and correctional facilities
  • Partner with treatment providers and healthcare systems
  • Develop referral networks for specialized services

Data Collection and Reporting

  • Collect and maintain accurate data using Results-Based Accountability (RBA) framework
  • Track participant enrollment, service delivery, and outcome metrics
  • Prepare monthly financial reports and quarterly performance reports
  • Document service delivery activities and participant progress
  • Participate in program evaluation and quality improvement activities

Administrative Functions

  • Attend required monthly vendor meetings with WPFC staff
  • Participate in quarterly site visits and program reviews
  • Maintain compliance with all program requirements and policies
  • Ensure proper documentation and record-keeping procedures
  • Coordinate with finance department on budget management

Required Qualifications

Education and Experience

  • Bachelor's degree in Social Work, Human Services, Psychology, Criminal Justice, or related field
  • Minimum 3 years of experience working with justice-involved populations
  • Experience providing services to individuals with mental health and substance use disorders
  • Demonstrated experience with case management and care coordination

Knowledge and Skills

  • Understanding of criminal justice system and re-entry challenges
  • Knowledge of mental health and substance use treatment approaches
  • Familiarity with trauma-informed practices and Adverse Childhood Experiences (ACEs)
  • Understanding of social service systems and benefit programs
  • Experience with Results-Based Accountability or similar outcome measurement frameworks

Core Competencies

  • Strong interpersonal and communication skills
  • Ability to work with diverse populations and address racial equity issues
  • Crisis intervention and de-escalation skills
  • Computer proficiency including data entry and reporting systems
  • Valid driver's license with clean driving record (mandatory)
  • Reliable personal vehicle or access to program vehicle
  • Ability to work flexible hours including evenings and weekends
  • Defensive driving certification preferred

Preferred Qualifications

  • Master's degree in relevant field
  • Certification in peer support services or similar credential
  • Experience with formerly incarcerated populations
  • Bilingual capabilities (English/Spanish)
  • Previous experience with grant-funded programs
  • Knowledge of Wicomico County service systems

Physical Requirements

  • Ability to work in various community settings including client homes, treatment facilities, and social service offices
  • Capacity to drive extensively throughout Wicomico County (potentially 50-100 miles daily)
  • Ability to assist participants getting in and out of vehicles safely
  • Ability to lift up to 25 pounds occasionally
  • Standing, walking, and sitting for extended periods including long driving periods
  • Visual acuity for safe driving in various weather and lighting conditions
  • Use of computer and office equipment

Working Conditions

  • Office environment and extensive community-based field work
  • Daily driving throughout Wicomico County in various weather conditions
  • Regular travel to meet participants in diverse locations including treatment facilities, social service offices, and court buildings
  • Some evening and weekend work required for appointments and emergencies
  • Exposure to potentially stressful situations during transportation and service delivery
  • Work with individuals in crisis or emergency situations
  • Potential exposure to various environmental conditions in client homes and community settings
