Family Services Coordinator

We are seeking a compassionate and skilled Family Services Coordinator to provide social work services to patients and families coping with terminal illness. This role supports our interdisciplinary team by offering counseling, advocacy, and case management to ensure the highest quality of care and support throughout the end-of-life journey.

· Master’s degree in Social Work from an accredited program (LCSW preferred)

· At least one year of healthcare experience; hospice experience strongly preferred.

· Strong understanding of hospice philosophy and end-of-life issues.

· Excellent written and verbal communication skills.

· Current CPR certification and ability to receive annual influenza vaccination.

· Valid Colorado driver’s license, insurance, and reliable transportation.

· Must meet company driving standards.
