Responsibilities
- Conduct face to face visits with assigned clients
- Identifies and resolves barriers that hinder effective care.
- Ensures client is progressing towards desired outcomes by continuously monitoring care plan
- Review and record patients medical histories
- Keep records of patients progress, or lack thereof
- Be able to relate with patients and their families
- Make necessary patient appointments
- Transportation of clients as needed
- Tracking and Report mileage of client transports
- Adhering to managers directives

Qualifications
- Reliable vehicle and appropriate insurance/registration for home visits.
- Must have a Valid driver’s license and the ability to travel within the state, as required
- Experience working in home settings with populations of diverse cultures and ethnicities preferred.
- Able to communicate well verbally and in writing.
- Comfortable with computers and experienced in writing detailed notes
- Strong organization skills

To ensure Home or Hybrid Home/Office associates’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office associates must meet the following criteria:
- At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested
- Public Wi-Fi cannot be used or considered as an option
- Satellite, cellular and microwave connection can be used only if approved by leadership
- Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
