At Seniornest Homecare, nurses lead the way in helping older adults age safely and comfortably at home. We’re seeking an experienced Registered Nurse (RN) to join us as a Field RN – Home Care Nurse Supervisor, combining hands-on patient care with clinical leadership in the field. In this role, you’ll manage a small caseload of home care clients, complete in-home assessments, build and update plans of care, and provide clinical oversight to our home health aides. Key responsibilities
- Conduct initial and ongoing in-home nursing assessments, including vitals and focused physical assessments, and translate findings into clear, updated plans of care.
- Develop, implement, and revise individualized care plans in collaboration with families and the office care coordination team.
- Perform field visits to monitor clients’ status, address clinical concerns, and ensure care can be safely delivered at home.
- Supervise, coach, and support home health aides in the field, including supervisory and orientation visits, skills reinforcement, and performance feedback.
- Educate clients and families on disease management, medications, safety in the home, and the plan of care, reinforcing teaching at each visit.
- Document all visits, assessments, and communications accurately and promptly in the electronic record, in accordance with agency and DOH standards.
- Ensure compliance with state and federal home care regulations and agency policies, escalating issues as needed to the Clinical Manager.
- Participate in the on-call nurse rotation to provide clinical guidance and handle urgent issues for field staff and clients.
- Collaborate with the office team on admissions, case conferences, quality improvement, and service recovery when client issues arise.
Qualifications
- Active New Jersey Registered Nurse (RN) license in good standing.
- Minimum 1 year of nursing experience; home care, home health, hospice, or community-based experience strongly preferred.
- Strong assessment skills, comfort working independently in the field, and ability to prioritize a visit schedule.
- Excellent communication skills with clients, families, caregivers, and providers.
- Reliable vehicle, valid driver’s license, and ability to travel between client homes within the service area.
