Job Title: RN Visit Nurse – Flexible Role with Independence

Compensation: $75,000 – $100,000 annually + $4,000 in Bonuses During First Year of Employment

What You’ll Do:

• Complete in-home assessments and update care plans as conditions evolve
  
• Perform skilled procedures such as wound care, med management, and injections
  
• Provide clear patient and caregiver education on care plans and chronic condition management
  
• Coordinate care with our interdisciplinary team and communicate with providers
  
• Maintain accurate documentation and regulatory compliance
  

Ideal Candidates Have:

• MN Registered Nurse license in good standing
  
• At least 1–2 years of nursing experience (home care or hospice a plus)
  
• EMR proficiency and strong clinical judgement
  
• Valid driver’s license and reliable transportation
