Responsibilities:
- Conduct comprehensive assessments to identify the needs, strengths, and goals of clients and their families.
- Develop individualized care plans and interventions to address clients' social, emotional, and practical needs.
- Provide counseling, advocacy, and support services to empower clients to overcome challenges and achieve positive outcomes.
- Coordinate and monitor the delivery of services and resources to clients, ensuring continuity of care and adherence to established protocols.
- Collaborate with interdisciplinary teams, including healthcare professionals, educators, and community agencies, to facilitate holistic support for clients.
- Maintain accurate and up-to-date documentation of client interactions, progress, and service utilization.
- Respond promptly to crisis situations and provide crisis intervention services, including assessment, de-escalation, and safety planning.
- Collaborate with emergency services, law enforcement, and other agencies to ensure the safety and well-being of clients in crisis.
- Engage with community stakeholders to raise awareness of social issues, resources, and available support services.
- Conduct educational workshops and presentations on topics related to mental health, substance abuse, domestic violence, and other social concerns.
- Participate in community events, fairs, and outreach activities to promote access to services and reduce stigma surrounding mental health and social issues.
- Participate in ongoing training, supervision, and professional development opportunities to enhance clinical skills and knowledge.
- Pursue licensure and certification as required by state regulations and professional standards.
Requirements:
- Master's degree in Social Work (MSW) from an accredited program.
- New graduates are welcome; previous internship or practicum experience in social work or related field is preferred.
- Strong interpersonal skills and the ability to establish rapport with clients from diverse backgrounds.
- Empathy, compassion, and a genuine desire to help others overcome challenges and achieve their goals.
- Excellent communication, problem-solving, and organizational skills.
- Ability to work independently and collaboratively in a fast-paced environment.
Please note- The annual base salary range is $75k to $90k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
