Job Description - Foster Care Caseworker - Washington, PA

Now Hiring: Foster Care Caseworker - Bachelor Degree in Human Services (including but not limited to: Psychology, Sociology, Criminology or other Social Services Degree)



Join our Team! Blueprints is hiring a Foster Care Caseworker.  The Caseworker will Represent Blueprints within the social services community and serves as a liaison in collaborative agency efforts. Maintain a caseload of clients, establish positive relationships with each client and foster family and ensure all program and treatment goals are being addressed and attained. Develop and implement an individual service plan for each assigned client through collaboration with foster care staff and placing agency representative.



WORK WEEK:



  • Monday – Friday 9:00 AM – 4:30 PM (Times/Days may vary)



BRIEF DESCRIPTION OF DUTIES:



  • Regular and reliable attendance is essential.

  • Understand and maintain compliance with contractual agreements and agency contracts with Children and Youth Services, to include completing all paperwork associated with the position and meeting deadlines as outlined within such contracts.

  • Maintain awareness of and abide by agency organizational structure, goals, mission, policies, and procedures and all local, state and federal regulations and requirements. Represent same throughout the community.

  • Implement assigned SWAN Units of Service as described by SWAN benchmarks.

  • Attend program meetings as requested by supervisor or program director.

  • Represent Blueprints and various programs within the social services community and serves as a liaison in collaborative agency efforts.

  • Maintain a caseload of participants, establish positive relationships with each and ensure all program and treatment goals are being addressed and attained.

  • Develop and implement an individual service plan for each assigned participant through collaboration with permanency staff and placing agency representative.

  • Monitor participant progress according to the individual service plan through regular contact and schedule progress sessions to collaborate with all participant and treatment members regarding continuing care or propose revisions in service plan when deemed appropriate.

  • Complete all required reports in a timely basis and maintain additional contact to inform the referring agency of participant progress as needed.

  • Attend court hearings and meetings scheduled by the referring agency pertaining to participants.

  • Provide crisis intervention and prevention 24 hours a day, seven days a week, through participation in on-call rotation as scheduled.

  • Develop a working knowledge of other community agencies and their services for the purpose of assessing a participant's suitability for services through these agencies.

  • Assist supervisor with training of foster parents and the development and continuation of monthly foster parent support groups.

  • Maintain face-to-face contact with each participant on assigned caseload as required by regulations, agency policy, and assigned by supervisor.

  • Re-evaluate current foster homes on a yearly basis as assigned by supervisor.

  • Provide in home training to foster families as assigned by supervisor.

  • Assist with additional caseload, participate in recruitment of foster parents, and provide supportive services to birth families of clients as needed.
