Occupational Therapist Assistant / Rehab Director (Full-Time)

Synergy Care is seeking a dedicated Occupational Therapist Assistant (COTA) / Rehab Director to join our team full-time at Winning Wheels, a skilled nursing facility in Prophetstown, Illinois. We welcome both licensed therapists and new graduates to apply.

Key Responsibilities

Patient Care & Treatment

• Implement treatment plans established by the supervising Occupational Therapist.


• Utilize therapeutic activities and exercises to improve patient function.


• Assist patients with activities of daily living (ADLs), including dressing, grooming, feeding, and mobility.


• Apply adaptive equipment and assistive technology to promote patient independence.


• Encourage and motivate patients throughout the rehabilitation process.

Documentation

• Maintain accurate, timely, and compliant treatment documentation.


• Record patient progress, goals, and outcomes for review by the supervising Occupational Therapist.

Collaboration & Communication

• Communicate regularly with the supervising Occupational Therapist regarding patient status and treatment modifications.


• Participate in interdisciplinary team meetings as appropriate.


• Educate patients, families, and caregivers on therapy techniques and home exercise programs.

Qualifications

To be considered, candidates must:

• Be a graduate of an accredited Occupational Therapy Assistant program (or graduating within six months).


• Hold a valid NBCOT certification.


• Hold or be eligible for state licensure as an Occupational Therapy Assistant.
