Mobile Health Specialist (Community Paramedic)

Mobile Health Specialist (Community Paramedic): Job Responsibilities
- Home Visits: Conduct in -person home visits to manage acute illnesses and injuries.
- Treatment & Collaboration: Provide treatment in line with evidence -based guidelines and collaborate with a multi -disciplinary team, including telehealth Emergency Physicians and care coordinators, to handle complex cases.
- Risk Assessment: Identify risk factors, such as medication compliance, limited mobility, impaired nutrition, fall safety, and social isolation.
- Documentation & Compliance: Maintain patient confidentiality, complete timely electronic records, and adhere to acute care standards.
- Vehicle Requirements: Respond on an on -call basis using a personal vehicle, with the option to use a Care2U vehicle when available and pick up necessary equipment from designated locations.
Compensation and Job Type:
- On -Demand: $40 per hour, with three hours of pay for the first patient and two hours per each additional patient (most responses average 75 minutes). Responses are as needed 365 days a year from 8 am to 10 pm.
