Responsibilities Include:

• Performing physical therapy evaluations and establishing care plans
  
• Teaching exercises and injury prevention techniques
  
• Helping patients adapt to physical challenges at home
  
• Charting progress and modifying care based on outcomes
  

Qualifications:

• Licensed Physical Therapist in Minnesota
  
• Outstanding communication and care coordination skills
  
• Reliable vehicle and driver’s license required
  
• Previous home health or rehab experience a plus
