FT Registered Nurse (RN) Care Navigator

Bishop Gadsden is seeking a compassionate Registered Nurse Care Navigator to help older adults and their families navigate healthcare decisions, coordinate services, and maintain the highest quality of life.

This is a unique opportunity for an RN who enjoys relationship-building, advocacy, and care coordination in a non-bedside role.

What You'll Do:

• Guide residents and families through healthcare and aging-related decisions
• Coordinate appointments, services, and community resources
• Develop and support individualized care plans
• Monitor resident needs and communicate changes in condition or level of care
• Complete assessments and Long Term Care insurance documentation
• Partner with families and interdisciplinary teams to ensure excellent resident support
• Accompany residents to medical appointments as needed

What You'll Bring:

• Active RN license in the state of SC
• 2+ years of nursing experience
• Knowledge of senior services and aging resources
• Ability to accurately and efficiently document care in a timely manner
• Strong communication, organization, and customer service skills
• Valid South Carolina driver's license and clean driving record for the past 3 years
