Job Description - Full Life Care - Clinical Service Coordinator

If so, look no further—Full Life Care, a Transforming Age Affiliate, is seeking a full-time Social Services Professional to provide social work consultation and coordinate client care. Our Clinical Service Coordinators: 



  • Provides client services including client skills training, exposure intervention, and other support services.

  • Collaborate with Clinical Director and ACHP Clinical Specialist to manage referral process, intervention plans, and intervention strategies. 

  • Meets regularly with ACHP team to ensure that staff, client and contract requirements are met with efficiency and maximum efficacy.

  • Screens ACHP referrals and effectively coordinates follow-up with Clinical Director and Clinical Specialist. Coordinates with other associates to ensure client home area is efficiently maintained

  • Coordinates care for those ineligible ACHP referred residents to the SHA case management programs, other healthcare providers, and SHA contractors.

  • Coordinates aftercare for ACHP clients to SHA case management programs, other healthcare providers, and SHA contractors.

  • Ensures continuity of care by communicating changes in client condition or circumstances to ACHP team members, and others (ensuring confidentiality where needed).

  • Perform other duties as assigned


 


What you Need to Succeed:



  • Bachelor’s degree or higher in Social Work, Psychology, or a related field or a minimum of 2 years of experience in human services, healthcare, mental health, or a related field.

  • Strong verbal and written communication skills.

  • Understanding behavioral health challenges, including:

    • Hoarding Disorder (HD)

    • High-cluttered environments

    • Functional limitations in daily living skills



  • Basic knowledge of:

    • Trauma-Informed Care

    • Harm Reduction principles

    • Motivational Interviewing



  • Ability to collaborate with clients of varying abilities to support:

    • Safe and healthy housing

    • Daily functioning

    • Social interaction and engagement





  • Proficient in Microsoft Office Suite (Excel, Word, PowerPoint, OneNote, Teams) and Adobe.

  • Comfortable working:

    • In-person or virtually with staff (via Teams/Zoom)

    • In clients’ private residences, including those with complex behavioral health needs or co-morbidities




 


Physical Requirements: 



  • Must be able to lift, push, and pull up to 50 lbs.

  • May occasionally need to assist with client mobility.
