Full Life Care - Service Coordinator, SHA Outreach

The Services Coordinator - SHA Outreach provide information and referral services to low-income and disabled adults residing in Seattle Housing Authority Low-income Public Housing portfolio. (LIPH). Services include connecting residents with behavioral health services and support, chemical dependency treatment services, accessing primary medical care, accessing rental assistance resources, enrolling with a representative payee and problem-solving issues which may negatively impact residents’ ability to maintain stable housing with Seattle Housing Authority. (SHA). Advocate for residents and connect with services in order to prevent resident evictions. Must adhere to Federal, State and County housing policies as well as Full Life Care rules and regulations.

Our Services Coordinators: 

• Receives and responds to referrals made by Seattle Housing Authority (SHA) property management and other community partners to provide services to elderly and non-elderly disabled residents.


• Works collaboratively with SHA residents and staff. Conduct on-site client needs assessments and develop mental health case management plans.


• Completes regular outreach to individuals residing within SHA’s LIPH public housing portfolio and maintain weekly office hours within LIPH buildings. Must have reliable transportation in order to complete outreach work and to offer services.


• Assists residents in maintaining a safe and healthy environment with education in hoarding/pest prevention; assist with treatment options.


• Assists residents with behavioral health needs to contact a behavioral health provider for services and supports.


• Advocates for and directly assist residents of low-income public housing communities with housing stabilization resources, including rental assistance programs, hoarding support programs, and pest control remediation services.


• Maintains an awareness of local, state and national resources that are available to SHA residents with food insecurity, transportation barriers, medical issues, education needs, domestic violence threats, and other life challenges.


• Complete trainings, projects and other tasks as assigned by Program Manager.


• Maintains a safe and healthy environment, adheres to infection control protocols; addresses potential safety and health hazards; follows emergency policies and procedures; fulfills education and certification requirements.


• Maintains quality client services by following standards and policies and implement program plans.


• Maintains required documentation to meet WAC and HUD guidelines, as well as Full Life Care policy.


• Performs other duties as assigned.

What you need to succeed! 

• Bachelor’s Degree in related field, i.e. Psychology, Sociology or Mental Health Studies, or a minimum of two years of related professional work experience.


• Must be certified in CPR and attend HIV/AIDS information training; if not currently certified in CPR and HIV/AIDS information training, must be willing to become so within 6 months of hire.


• Understanding of trauma care, harm reduction, motivational interviewing

Physical Requirements for the role: 

• Must be able to lift, push, and pull up to 25 lbs, may occasionally need to support client mobility.
