We are hiring a Social Services Professional to help support clients’ well-being and independence by providing social work consultation; coordinating client care, as well as develops and facilitates therapeutic group activities. 

Our Social Services Professional: 

• Provides information to referral sources and potential clients by receiving intake inquiries and giving tours.


• Identifies clients’ service needs by gathering and completing intake information and working with the client, caregiver and/or referral source to develop service plans.


• Provides team with social work expertise.


• Helps maintain continuity of care among team members by providing communication, documentation, and support. 


• Encourages enrollment by maintaining contact with caregivers, referral sources and potential clients as well as site liaisons and site managers from other sites.


• Maintains client confidence and protects operations by keeping information confidential.


• Develops a safe, therapeutic group environment by establishing and keeping group rules; monitoring and encouraging appropriate participation; intervening as necessary.


• Maintains required documentation to meet WAC guidelines and/or Full Life Care policy;
  


• Performs other duties as assigned.

What you need to succeed! 

• Master’s degree in social work, gerontology, or other human services field, or counseling and at least one year of professional work experience (full-time equivalent), or a bachelor's degree in social work, counseling, or a related field and two years of experience in a human services field, or related field.


• Must complete the HIV/AIDS information course;


• Employees are required to receive an annual influenza vaccine unless granted a medical or religious exemption.

Physical Requirements for the role: 

• Must be able to lift, push, and pull up to 25 lbs, may occasionally need to support client mobility.
