Job Title: Registered Nurse

Job Overview:

As RN of Luminos Hospice, you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best-in-class hospice care.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Other duties as assigned.

• RN license in the state of Texas
• Current driver’s license, reliable transportation, valid auto insurance
• Current CPR and BCLS certification
• Excellent verbal and written communication skills
• Two (2) years experience as a registered nurse in a clinical care setting, hospice preferred
