Full Time RN Case Manager

Our chosen candidate will support our patients and families in the Charlottesville and Augusta County areas.

Job responsibilities can include but are not limited to:

• Administer skilled nursing for all ages in their place of residence.


• Conduct patient admissions.


• Conduct routine patient visits.


• Coordinate care within a hospice interdisciplinary team to assure patient/family needs are met.


• Communicate with attending physicians, hospice physicians, and other physicians involved in the patient's care.


• Demonstrates accountability, flexibility, respect, and integrity.


• Submits all required paperwork and documentation within 48 hours. 

Qualifications:

• Current Registered Nurse license in the state of Virginia. .


• One (1) years experience in a hospice setting heavily preferred (but not required) as a RN Case Manager or similar role. 


• Must have a current Texas Driver's license and dependable transportation.
