Location: Inpatient Facility - Saranac Lake, NY

Position: Full-time Aftercare Counselor

Shift/Schedule: Tuesday-Saturday, 8:00am-4:30pm 

Pay Range Levels:

• Counselor without a credential: $45,000.00/year


• CASAC-T / CASAC-P: $47,008/year to $49,004/year


• CASAC to CASAC - Masters: $58,500/year to $72,000/year


• MHC / MSW: $67,000/year to $68,000/year


• LMSW / LMHC: $75,000/year to $100,000/year

Position Summary: Under the direct supervision of a QHP, facilitates completion of necessary paperwork/applications, screenings and interviews with Counselors and residents for Aftercare placement.

Education and Training: High school graduate or equivalency, AAS preferred, CPR certification

Qualifications and Requirements: Telephone skills, typing skills, standard business practices, able to work independently, computer skills and knowledge of chemical dependency helpful.  CASAC preferred.

Essential Duties:

• Develop and maintain a therapeutic relationship with clients.


• Actively participate in clinical supervision.


• Function as a supportive multi-disciplinary team member.


• Complete thorough admission procedure for clients.


• Assists with program service delivery in absence of Primary Counselors to include admissions, assessments, treatment plan coordinator and group/individual coverage.


• Coordinating Resident transportation for discharge.


• Assist Residents in completing Aftercare referral applications.


• Conduct aftercare sessions.


• Perform discharge meetings.


• Compiling and maintaining of referral files.


• Collaborating with the Utilization Review Team for coordination of resident funding in preparation for discharge.


• Responsible for facilitation if didactic lectures as assigned.


• Facilitate phone screenings.


• Follow up with referrals to establish placements for aftercare.


• Responsible for additional duties as may be assigned by the Supervisor.


• Display professional image, appearance, and behavior reflective of SJRC Mission, policies and procedures.


• Provide weekend and holiday coverage.
