Home Hemodialysis Registered Nurse (RN) - Patient Care Specialist

Essential Responsibilities:

• Deliver expert home hemodialysis treatments while ensuring compliance with clinical protocols
• Provide comprehensive patient assessment, monitoring, and care plan modifications
• Lead patient and family education programs for home dialysis procedures
• Coordinate care with nephrologists, dietitians, and other healthcare team members
• Implement and maintain rigorous infection control protocols
• Document treatments, medications, and patient outcomes in electronic health records
• Respond to patient emergencies and provide on-call support as scheduled

Required Qualifications:

• Active Registered Nurse license in Indiana
• Current BLS certification
• 1+ year of nursing experience
• 6+ months dialysis experience preferred
• Strong communication and interpersonal skills
• Valid driver's license and reliable transportation
