- $140,000 to $180,000+ Base Salary, dependent upon experience

POSITION DESCRIPTION: 
The Stroke Program Nurse Practitioner provides expert nursing consultation and practice oversight, functioning as a liaison between medical staff, nursing staff, family members, and support services.

Specific duties of the role include, but are not limited to: 

- coordinate multidisciplinary care
- provide stroke education
- provide delivery and oversight of patient care
- post-discharge follow-up
- work in collaboration with key stakeholders to meet program certification requirements
- review data and performance improvement projects
- incorporate the patient experience in all aspects of patient care   

The organization uses an EHR.


REQUIREMENTS:
- California NP license
- DEA
- previous stroke experience
- new grads with RN backgrounds in stroke care are welcome to apply
