Primary Responsibilities:

• Evaluate client function and mobility through in-home assessments
  
• Design and implement patient-specific rehab strategies
  
• Provide hands-on therapy, exercise instruction, and equipment recommendations
  
• Collaborate with nurses, aides, and family caregivers
  

Requirements:

• Current MN license as a Physical Therapist
  
• Valid driver’s license and vehicle for travel
  
• Experience in home health preferred, but we welcome outpatient/inpatient backgrounds
  
• Passion for client-centered rehabilitation
