Job Summary

The Field Nurse Supervisor (RN) oversees the day-to-day clinical operations of our home care field team. This role includes supervising nurses and caregivers, performing in-home assessments, developing care plans, and ensuring quality standards and regulatory compliance. You’ll also serve as a key clinical liaison between clients, families, and our care coordination team.

Key Responsibilities

  Conduct patient assessments and ensure accurate, up-to-date care plans.

  Supervise field home health aides to ensure delivery of quality care.

  Provide clinical training, coaching, and performance feedback for field staff.

  Perform home visits to monitor patient care and resolve clinical concerns.

  Ensure compliance with state and federal home care regulations.

  
  


• 
  

  Participate in the on-call nurse rotation for clinical support.

  
  

Qualifications

• 
  

  Active NJ Registered Nurse (RN) License required.

  
  


• 
  

  1+ years of nursing experience (home care, home health, or supervisory experience preferred).

  Excellent communication, leadership, and time management skills.

  
  


• 
  

  Strong documentation and organizational skills.
