Home Health RN for Skilled Visits to service San Antonio and surrounding areas.Full-Time / PRN :$75.00 per OASIS assessment/recert visits.

$50.00 per Non-OASIS assessment/recert visits.

$45.00 per treatment visits.**A tablet will be provided for all Full-Time RNs's.

What You'll Do:

Conduct comprehensive assessments of patients' medical conditions and needs. Develop and implement individualized care plans in collaboration with the team. Administer medications, treatments and procedures as prescribed by physicians. Monitor patients' responses to treatments and make necessary adjustments to care plans. Maintain accurate and timely documentation of all patient interactions, treatments, and progress. Communicate effectively with patients, families, and other healthcare providers to ensure coordinated and optimal patient care.

Qualifications:

Current and valid Registered Nurse (RN) license in the state of Texas

CPR & BLS certification. Strong clinical skills and knowledge of home health care practices. Ability to work independently and manage time efficiently.
