Capstone Hospice is in search of a Social Worker to join our dedicated hospice team covering the territory of North Gwinnett areas of Suwanee / Lawrenceville. This position is full-time.
The Social Worker provides a wide range of psychosocial services to terminally ill/chronically ill patients and their families including regularly assessing the patient and family's psychosocial needs, pre-bereavement/anticipatory grief needs and coordination of care. The position works in the field providing these services in residential care, assisted living facilities, personal care homes and private home setting. This position will report to the Social Services Manager.

Candidate Application Process:

After an in-person interview, candidates for consideration will be subject to a background check, pre-employment physical and drug screening.
Candidates must meet the following requirements to be considered:
· Licensed degree in social work; LMSW or LCSW.
· Minimum 2 years of experience as a social worker.
· Hospice experience preferred.
· Acceptance of and adaptability to diverse family environments, different socio-economic, cultural, racial and religious modes.
· Must have current valid driver's license, auto liability insurance and reliable transportation.
· Must provide evidence of annual TB test, COVID vaccination at time of hire.
Candidates should possess the following abilities to be successful in this role:
· Ability to work independently.
· Ability to make accurate, and at times quick judgements.
· Ability to respond to crisis outside of an office/hospital setting.
· Ability to work within an interdisciplinary setting as part of a team.
· Ability to be flexible, innovative and possess good interpersonal skills.
· Ability to cope with mental and emotional stress and demonstrate emotional stability.
Essential functions of a hospice social worker:
Provide a wide range of psychosocial services to terminally ill/chronically ill patients and their families including regularly assessing the patient and family’s psychosocial needs, Pre-Bereavement/anticipatory grief needs and coordination of care. Providing education, outreach and referral as needed.
Create and implement care plans to meet needs which include, but are not limited to:
Working Conditions:
Be able to tolerate exposure to elements, including, but no limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.
Physical Requirements:
Requires minimal physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
Mental Requirements:
Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.
This is a full-time position with the expectations of a 40-hour work week.
