Job Description - Functional Screen Quality Specialist

Position Summary:

Under the direction of the Functional Eligibility (FE) Manager, the Functional Screen Quality Specialist (FSQS) performs duties directly related to improving or maintaining Long Term Care Functional Screen (LTCFS) quality for Lakeland Care Inc.’s Family Care Program. This includes but is not limited to conducting quality reviews of the LTCFS, evaluating inter-rater reliability, implementing improvement efforts, developing training opportunities for a variety of audiences, and determining validity of rescreen requests. This position will serve as a subject matter expert on the application of the LTCFS. 

 Responsibilities & Competencies:

  • Develop, maintain, evaluate, and deploy LTCFS quality tools, metrics, and systems on an ongoing basis. 
  • Understand quality management principles, including describing the principle, explaining its importance, determining the benefits of the principle, and developing action steps to improve the organization’s performance when applying the identified principle.
  • Measure, translate, and share findings throughout the organization while identifying system impacts.
  • Support LCI’s Family Care Program by reviewing LTCFS, consulting, and providing guidance to promote optimal application of the LTCFS Instructions. 
  • Identify current or potential future problems. 
  • Generate solutions that can be acted upon that will not just mitigate the problem; ensure the root issue is addressed. 
  • Identify innovative ideas and solutions while being mindful to personal biases.
  • Review the validity of rescreen requests and prioritize accordingly.
  • Aid in the identification, development, and implementation of training opportunities necessary to enhance understanding and application of the LTCFS.
  • Conceptualize ways to improve the quality and effectiveness of the department. 
  • Be proactive in personal education. 
  • Demonstrate ability to manage workload needs by prioritizing work/life harmony.
  • Provide ongoing and effective feedback to FES surrounding their application of the LTCFS Instructions and the quality of LTCFS. 
  • Invite peers into the thought process to generate new perspectives and point out blind spots. 
  • Effectively share ideas and solutions. 
  • Create an atmosphere of respect, helpfulness, and cooperation. 
  • Give and receive feedback from peers and/or team members.  
  • Acknowledge others’ skills, experience, creativity, and contributions. 
  • Seek understanding by asking questions for clarification.

  • Demonstrate ability to set judgement aside, when necessary, to facilitate strong interactions with colleagues.
  • Work collaboratively with the Functional Eligibility Manager to conduct targeted audits of LTCFS. 
  • Partner with the Functional Eligibility Supervisors to build LTCFS competencies of the Functional Eligibility Specialists (FES).  
  • Serve as a co-lead with the Functional Eligibility Manager for LCI’s LTCFS Quality Program.
  • Build productive working relationships with coworkers. 
  • Demonstrate strong emotional intelligence through reactions and responses to challenging situations.
  • Maintain an understanding of LCI’s business operations and how LCI achieves goals and objectives. 
  • Think strategically and tacitly with a focus on business results.
  • Apply knowledge to solve business problems by collecting and sharing relevant information.
  • Effectively communicate with a broad audience of internal LCI stakeholders across multiple divisions.

Requirements

Qualifications:

  • Bachelor’s degree, preferably in a health or human services related field, or have a license to practice as a registered nurse in Wisconsin pursuant to Wis. Stat. § 441.06.
  • Minimum of three (3) years professional experience in a long-term care organization or community-based setting preferred; at least one year of professional work experience with one or more of the target groups served by Family Care is required.
  • Strong knowledge of the Wisconsin Long Term Care Functional Screen.
  • Possess current State of Wisconsin Long Term Care Functional Screen certification; or will pass the online LTCFS Training Course within 30 days of hire.
  • Completion of all mandatory certification courses, exams, refresher courses, and continuing skills testing as required by the Department of Health Services (DHS) to maintain LTCFS Certification.
  • Ability to travel throughout all regions served by the organization and throughout the state of Wisconsin. 
  • Current driver’s license, acceptable driving record and proof of adequate insurance required.
