Job Description - Gap in Care Coordinator

Qualifications: Must have a high school diploma or GED.



Days/Hours: Monday - Friday,40 hours a week



Position Summary: To act as a go-between with managed care companies, reviewing Gaps in Care reports to identify patients who have not been seen in a few years, have missed labs and screenings, and are frequent ER utilizers. This level of monitoring of the patient panel will result in better patient follow up with fewer ER and office visits and improvement in quality-of-care indicators.


 
Knowledge/Skills: Previous health care knowledge is desirable but not mandatory. Must have good working knowledge of Microsoft Word, Microsoft Excel, etc. Must be able to sit for extended periods of time and must also be able to tolerate extended periods of time in front of a computer screen.



Job duties include but are not limited to: 




  • Review Gaps in Care reports to identify patients who have not been seen in two years, have missed labs and screenings, and are frequent ER utilizers.

  • Schedule all CC appointments according to the spreadsheets or task messages or ACC.

  • Share information with the management team regarding results of Gaps in Care reports and other clinical indicators.

  • Review Records for GAP closures and Request Records if needed.

  • Add appropriate Billing codes in order to close GAPS.



  • Help develop reminders and other tools to prompt and inform staff of what patients are due for.



  • Work with Care Coordinators to supply them with information needed to contact patients and work reports.

  • Meet with Chief Revenue Officer on a regular basis to discuss efforts, best practices, and how claims are being paid for care coordination services/efforts.

  • Perform any other duties as assigned by your supervisor.


 





    ***Salary scales are a range of pay and offers are made to applicants based upon the requirements of the particular position. Applicants with “previous like skills” may not be given the same weight and consideration as “previous specific same skills”: i.e., For a Medical Receptionist position, “Previous clerical skills” may not be preferred over “Previous Medical Clerical Skills”
