Geriatric LPN/LVN ( Per Diem)

Location: Simi Valley, CA and Surrounding Areas. 

Schedule: Per-Diem 

Rate: $45-80 per visit based on experience and qualifications.

Position Overview:
The Licensed Practical/Vocational Nurse (LPN/LVN) provides compassionate and skilled nursing care to clients as prescribed by their Physician, under the guidance and supervision of a Registered Nurse (RN). This role is performed in accordance with the State LPN/LVN Practice Act, applicable licensure requirements, and the policies and procedures of Remedy Home Health Care.

Key Responsibilities:

• Deliver direct nursing care to clients based on their individual needs and care plans.


• Work collaboratively with the care team to ensure optimal health outcomes.


• Maintain detailed, accurate, and timely documentation of all client care and activities.


• Uphold agency policies, procedures, and state regulatory guidelines.

Qualifications:

• Graduate of an approved School of Practical/Vocational Nursing.


• Active and current LPN/LVN licensure in good standing within the state(s) of practice.


• At least one (1) year of professional nursing experience (preferred in home health care).


• Physical ability to perform job duties, including standing, bending, stretching, and walking.


• Emotional stability and professionalism to work effectively with clients and families.


• Ability to communicate effectively in the client’s language and document all care-related activities.


• Compliance with educational and experience requirements outlined by state laws.


• Drivers License and reliable transportation
