👩‍⚕️ Your Role: 

As a Senior Doc Provider, you'll care for seniors where they feel most comfortable—their homes, assisted living residences, and care communities across the San Fernando Valley. You’ll be responsible for: 

• Conducting patient visits with care, efficiency, and clinical excellence 

• Managing chronic conditions and coordinating care across settings 

• Collaborating with a supportive clinical and administrative team 

• Providing consistent, compassionate communication to patients and families
