It’s a rare moment in time for dementia care – Centers for Medicare and Medicaid Services (CMS) recently announced an innovative payment model (Guiding an Improved Dementia Experience, GUIDE) that brings much-needed care to patients living with dementia and transformative support to their family caregivers. Under this new model, Medicare beneficiaries are eligible to receive these services at no cost to them. As we work to democratize access to world-class clinical care for patients and comprehensive caregiver support, we are looking for someone with experience implementing and managing government programs. Our Implementation Director will help us prepare for and oversee our participation in the newly announced GUIDE model – a once-in-a-lifetime opportunity to change the standard of dementia care and help millions of people around the country.
What you’ll do
Serve as our internal GUIDE expert; deeply understand the payment model, its requirements and any regulatory changes that may affect our participation in the program
Oversee program implementation and our ongoing participation in the program
Manage our partner ecosystem end-to-end; identify appropriate partners, build relationships with those partners, curate and refine our referral inventories (of community-based resources, respite organizations etc) and manage the requisite negotiation/contracting process
Work collaboratively across our clinical, operations, product and growth teams to prepare for implementation ahead of program go-live and achieve program objectives once the program is in-flight
Ensure compliance with all program requirements,program integrity standards, and reporting requirements
Build out program performance measurement processes across the following domains: clinical quality, patient experience, cost, utilization and health equity. Monitor and track performance measurement on an ongoing basis across these domains for internal and external reporting
Have 8+ years in healthcare program management or clinical operations
Have experience negotiating and contracting with vendors in healthcare
Have strong project management skills; you possess an innate understanding of how to break down a complex project and manage all parts of it to completion
Are obsessed with details and highly organized
Have your own engine. You see what needs to be done and raise your hand, versus needing to be told what to do and how to do it
Are an expert communicator and have excellent written/verbal communication skills
You’re the ideal candidate if you
Have experience working with vulnerable populations, particularly older adults or those with cognitive disorders
Have experience working in value-based care settings or on care delivery innovation
Have experience implementing and/or managing government programs in healthcare such as MSSP, CKCC, Direct Contracting, PCF, CPC+, BPCI, PACE
Are deeply unsatisfied with the pace of work at traditional healthcare organizations
