Are you passionate about making a difference in the lives of individuals with complex health needs? Delta Dental of Iowa is seeking a Government Programs Outreach and Care Coordination Specialist to lead care coordination and case management efforts for Government Programs members who have been identified as high-risk or those with complex oral and overal health needs. In this impactful role, you will lead, implement and support projects focused on care coordination, outreach, performance improvement, and member engagement, ensuring alignment with Government Programs contracts.
You will collaborate with key stakeholders including managed care organizations, the Iowa Department of Health and Human Services, I-Smile Coordinators, healthcare providers, and social workers. You will also work with our customer service team to identify member’s needs and get them linked to care. From facilitating mobile dental clinics and member advisory groups to assisting with planning and implementing health education campaigns and collaborating with community partners, this position is at the heart of promoting oral health access and advocacy.

Essential Functions and Principal Accountabilities:
Requirements
Required
1. Bachelor’s degree in health education or public health, public administration, social work or a related field of interest required.
2. Five years’ experience in a nonprofit/public health or social work position.
3. Experience with collaboration, coalition building and project management skills.
4. Excellent verbal and written communication and interpersonal skills.
5. Basic knowledge of Iowa’s human service and public health agencies.
6. Time management skills and flexibility to perform well under time pressure and with shifting priorities.
7. Ability to translate and follow contract language, federal regulations, and policies.
8. Proficient in use of Microsoft Office applications (Word, Excel, PowerPoint).
9. Ability to work independently.
10. Ability to travel is required in state 20%.
11. Ability to work flexible hours, including evenings and weekends, is required.
12. Must possess a valid State of Iowa driver’s license.
Preferred
13. Master’s degree with emphasis in social work or public health preferred.
14. Licensed to perform social work in the State of Iowa preferred.
15. Dental and/or insurance knowledge preferred.
16. Bilingual candidates (English and Spanish) preferred.
