Job Description - Group Home Coordinator

Fraser is currently hiring a Designated House Coordinator to join our Community Living team.  As a DHC, you will provide site-specific supervision and leadership to assure continuous quality improvement in active treatment service development and implantation at one of our Supervised or Supportive Living locations.


Requirements:


Candidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.



  • HS Diploma or Equivalent AND four (4) years of full-time work experience (8,000 hours) providing direct care services to persons with disabilities or nursing/assisted living home experience

  • BA/BS in Human Services or related field AND 1 year of full-time work experience providing direct care services to persons with disabilities 

  • Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities 


Responsibilities:



  • Manage staff coverage to meet the needs of the clients served

  • Coordinate completion of all household repairs and other upkeep

  • Coordinate the activities of daily living including personal care, hygiene, and grooming

  • Provide direct cares to our clients

  • Assure documentation for all areas of client care are completed 

  • Collaborate with medical professionals to coordinate health and medical needs for each client

  • Coordinate weekly, meal planning, and nutrition for clients

    • Golden Valley, MN 

    • Maple Grove, MN 



  • This position is full time Monday-Friday, standard business hours

  • The starting pay range for this role is $21.50 - $23.50 with an additional 2-dollar differential per hour depending on up qualifications and location 

  • $1,000 Hiring Bonus for external applicants
