Summary:

The Harm Reduction Outreach Coordinator plans, organizes, and operates a Syringe Services Program (SSP) and delivers person-centered, nonjudgmental harm reduction services in the community. This role provides Peer Support Services, builds and maintains strong relationships with local community partners, and coordinates outreach activities and community events to increase access to safer-use supplies, education, and referrals.

The Coordinator is responsible for writing and administering grants, ensuring accurate and timely data collection, and maintaining complete program documentation to support grant compliance and reporting requirements. Additional responsibilities include tracking service outcomes, preparing reports, and supporting other program needs and duties as assigned.

Description:

·         Promote and oversee Harm Reduction Syringe Exchange (HRSEP) program operations, including maintaining compliance with any existing local or state regulations or statutes. 

·         Work with local community partners and resources to increase participation in the HRSEP.

·         Provide Peer Support Services to clients seeking recovery and those who are actively in recovery including those who use medication for opioid use disorder (MOUD).

·         Use relevant personal stories to teach through experience and serve as a role model for clients and their families.

·         Provide group meetings as needed to support those in recovery.

·         May provide transportation of clients to medical appointments, recovery center, etc… deemed necessary on a case-by-case basis, with approval from direct supervisor or Public Health Director.

·         Work with community partners to support those in activity addiction and recovery.

·         Plan and provide community outreach events to facilitate and conduct NARCAN trainings and distribution, HIV/Hep C testing as well as any other public health related task that cultivates a proactive approach to substance use disorder, mental health, and over-all health and well-being within Bourbon County.

·         Track all Syringe Service Program (SSP) and outreach activities for reporting requirements.

·         Complete Redcap reporting for HRSEP, Narcan and Fentanyl Test Strips as required by program standards.

·         Complete required quarterly and annual service/program reporting to Bourbon County Fiscal Court and City of Paris for continued funding support for this position through the Opioid Abatement funds.

·         Assist with grant writing and grant reporting to help sustain SSP and barrier relief supplies to maintain program, (e.g., ASAP Traditional, ASAP Harm Reduction, KDPH Harm Reduction Expansion grants, others as needed.)

·         Participate and positively represent BCHD in community meetings, e.g., ASAP, HHS -ATOD and Mental Health committees and all other meetings as needed/assigned.

·         Develop and implement a public PR campaign to continually promote substance use awareness/treatment needs/community response/reduce stigma.

·         Represent the programs at Fiscal Court/City Commission meetings and all other meetings as needed.

·         Work with the KY Dept for Public Health always to promote programs/complete program requirements/develop a positive working relationship.

·         Obtain Maintain Certified Peer Support Speciation certification and obtain necessary CEUs annually.

·         Other duties as assigned.

Must have proficiency in oral and written communication and possess and certain level or relatability with the public.  Must be able to manage time and multi-task and navigate a face paced environment.  Must be able to work well with others as well as manage tasks on an individual basis.

MINIMUM REQUIREMENTS: 

High school diploma or GED and an up-to-date Certified Peer Support Specialist (or obtain with-in first 3months of employment); Valid Driver’s license and car insurance

WORKING CONDITIONS: 

Employee will report to the health department everyday Monday- Friday.  Work is 100% split between in office or community outreach (No work from home option).  Oversee all SSP operations, evaluating, connecting, and referring clients to treatment and/or harm reduction services. Employee will provide street outreach in the community by setup up information booths to provide Narcan training and distribution and other Harm Reduction information.  Employee may be located inside and outside depending on location and weather but will be provided with tent, table and chair and all supplies needed for outreach.  Also, support will also be in the office setting with clients and continuously keeping data collection.

PHYSICAL REQUIREMENTS / JOB HAZARDS

Current TB skin test indicates no evidence of communicable disease. Possible physical management (lifting, balancing, maneuvering).

Additional Requirements:

  • This position requires a negative drug screen prior to hire.

  • Must possess a valid driver’s license.

  • Must have reliable transportation.
