Cornerstone Family Healthcare is actively recruiting for a Harm Reduction Specialist to join our growing team in Newburgh.

RATE OF PAY/SALARY: $24.72 per hour

WORK LOCATION(S): Newburgh

STATUS: Full-Time/ Temporary

General Purpose:

Under the supervision of the Program Supervisor, the HRS Specialist will be responsible in identifying and linking people who inject/use drugs (PWID/PWUD) to harm reduction services offered by the Health Hub program to increase access and availability of buprenorphine within Orange County. The position is based in the Newburgh office.

Description of Duties:

  • Maintain program and agency standards as outlined in policies and procedures. 
  • Observe and abide by HIPAA and the HIV Confidentiality Law.
  • Assist clients schedule transportation for medical appointments and troubleshoot barriers to successful linkage to and retention in care. Escort clients as needed to appointments to assist in treatment adherence. 
  • Provide referrals to harm reduction services, drug treatment, entitlements, medical care, behavioral health and other services where applicable.
  • Conduct continual follow-up of clients who have received referrals.
  • Ensure that services are trauma-informed, based on a harm reduction model that is culturally responsive in order to produce positive outcomes for clients.
  • Continually review and assess the demand for services in the communities where the programs are located and adjust and/or develop programming as needed.
  • Encourage and maintain relationships with community-based resources helpful to the program’s priority population.
  • Provide street outreach to engage at risk individuals in to our services. 
  • Assess biopsychosocial needs, identify barriers to successful linkage, and connect clients to support services as appropriate through completion of screening, intake, and assessment.
  • Responsible for quality assurance activities, including timely completion of data entry in to AIRS, maintaining client charts within program standards and ensuring compliance with goals and objectives in the annual workplan.
  • Complete the mandated continuing education trainings as selected and scheduled by the Program Supervisor.
  • Participate in individual supervisions and attend department meetings. 
  • Assist in program coverage as needed.
  • Attend and participate in monthly department, All Staff and other required meetings. 
  • Be      familiar with Cornerstone policies and procedures and the Employee Handbook. 
  • Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations. 
  • Perform other related duties as assigned.

Requirements

  • High school diploma, or has passed either GED test or TASCTM and has earned a high school equivalency credential, or has 3 years’ experience in case management and related supportive services to PWUD, including 1 year of HIV/substance use-related experience.
  • Minimum of one-year experience working with PWUD, knowledge of HIV/AIDS, harm reduction, mental health and/or      community resources.
