HBS Homemaker

The nature of the programmatic and modality work performed by this position includes Home Based Services, Adoption and Guardianship Support Services, Family Preservation Services, and Family Centered Treatment services.  This role will assess clients assigned for services to determine the appropriate treatment plan; teach and assist clients in applying effective life skills; document all contacts with clients; and, produce all required reports in a timely fashion.

• Provide Adoption & Guardianship Support Services as needed and as assigned.
• Provide care according to evidence-based practice standards.
• Provide resources in the community to aid the children and family’s mental health and behavioral health, including support groups.
• Meet with clients on a regular, consistent basis. The frequency of visits is determined by the level of care needed on a case-by-case basis and/or by the referral source.
• Ensure the safety of the children and family through routine home and safety checks.
• Assess children and families’ needs at the beginning of services using approved assessments.
• Engage with the clients and use the assessments to create a service/prevention plan.
• Advocate for the family and be an active part of the family team.
• Make necessary referrals of community resources for clients.
• Coordinate and case manage appropriate educational, vocational, recreational, and other services, as needed.
• Completion of clear, accurate documentation of 1.) all contacts with clients, 2.) monthly reports, 3.) court reports, and 4.) treatment plans.
• Demonstrate respect and cultural sensitivity to all clients and to all referral sources.
• Maintain strict confidentiality at all times to protect clients' privacy.
• Comply with required in-service/continuing education hours.
• Lead and perform job duties that align with Lifeline’s core values and tenets of culture.
• Maintain accurate and timely records, reports, and data entry according to program standards.
• Demonstrate knowledge and skills required to successfully implement the program’s objectives.
• Participate in organizational in-service meetings as required.
• Attain/maintain CPR and First Aid certifications
